Payment |

You are
responsible for charges that are not covered by your
insurance, including any co-payment, at the time of your
appointment. If necessary, our insurance and billing
coordinator will assist patients to set up a payment plan.

Cancellation Policy |

Time has been specifically reserved for your physician appointment, procedure or treatment.
In order to be respectful of the needs of our patients, please be courteous and
call our
office promptly at least 24 hours ahead of time if you are unable to attend an appointment.
If you do not reach the receptionist, you may leave a detailed message on the voice mail.
This time will be reallocated to someone who is in urgent need of treatment. There is a $15
charge if you fail to show up for a scheduled appointment or cancel with less than 24 hours
notice.

Referrals |
|
|
Diagnosing and treating the symptoms of allergy and asthma is a medical specialty. Many insurance
companies require authorization, in the form of a referral, from your primary care
physician in order to see a specialist. If your insurance company requires a referral,
please obtain one for allergy/asthma symptoms, testing, and treatment to include at least three
office visits.
